Care Assessments
A Personal Care Assessment or Attendant Care Needs Assessment examines the level and quantity of assistance an individual needs in order to complete personal care tasks in his or her environment. In addition to the care assessment, treatment recommendations are provided in order to assist individuals in resuming effective independent completion of personal care tasks (where possible).
Home Safety Assessments
The Home Safety Assessment examines home accessibility following injury or illness. The home safety assessment identifies potential hazards in an individual’s environment to eliminate the risk of falls, re-injury, or aggravation of present injury.
Care Home Assessments
The Home Management Assessment examines an individual’s capacity and tolerances to complete tasks in the home (e.g., meal preparation, cleaning, outdoor tasks) while the home functional assessment examines all areas of function (self-care, productivity, and leisure). Performance components are assessed and analyzed in order to determine the level and quantity of assistance an individual requires to complete tasks in his or her environment and in order to establish treatment recommendations in order to assist the individual in resuming their daily occupations.
