Hospital Discharge Planner
Hospital Discharge Planning provides a link between the hospital and home/community environment. The Hospital Discharge Planner can work collaboratively with the family and hospital team members in order to assist with the discharge from hospital by ensuring an appropriate discharge environment, ensuring equipment needs are in place, and facilitating referrals to required community resources.
The Home Reactivation program aims to restore an individual’s function within the home. Following an examination of an individual’s capacity and tolerances with regard to self-care, productivity, and leisure, an individualized program is developed to assist the individual in gradually resuming pre-injury/ pre-illness roles and responsibilities. Treatment may include provision of assistive devices, education regarding proper use of posture and body mechanics, education regarding energy conservation, pacing, and joint protection. The individual may be supported in developing a schedule that encompasses the principles that have been taught and gradually re-introduces tasks as functional tolerances improve.
